PHYSICAL EXAMINATION

  • Students registering for Athletics must have a current physical examination form submitted for review before the student is authorized to participate in practice or tryouts. The Physical Examination shall not be taken earlier than May 1 preceding the school year for which it is applicable. ATHLETICS FORMS

  • Athletics forms must be completed and submitted online before a student will be allowed to practice and participate in any sport, cheerleading or dance team activity.

    Physicals are required by KSHSAA annually and shall not be taken earlier than May 1 preceding the school year for which it is applicable.
