PHYSICAL EXAMINATION

  • Students registering for Athletics must have a current physical examination form submitted for review before the student is authorized to participate in practice or tryouts. The Physical Examination shall not be taken earlier than May 1 preceding the school year for which it is applicable.   • 2025-2026 Sports Physicals & Paperwork Update

    Blue Valley is transitioning to a new athletics/cheer registration system which will not be available until June 1, 2025.

    Please wait until the new system is live on June 1 to submit your paperwork.  We will provide detailed instructions once the new system is ready. To be eligible for any KSHSAA activity during the 2025–26 school year, physicals must be dated after May 1, 2025.  

    Please download and print the physical form; it must be filled out and signed by a doctor.  You can access the official KSHSAA physical form for 2025–26 here
